Juntos de la Mano Program (Together Hand in Hand)
Juntos de la Mano, or "Together Hand in Hand," is a community resource program led by Community Health Workers (CHWs) at the Center. This initiative is designed to support Sonoma County's senior residents by connecting them to vital resources and services, such as food, healthcare, mental health, housing, and senior-specific support.
Led by trusted CHWs, Juntos de la Mano empowers seniors and caregivers with education, support, and easy access to services. Our CHWs bridge cultural and language gaps to ensure everyone receives the care they need. By creating meaningful connections, Juntos de la Mano is building a more equitable, accessible network of care for our community.
Sonoma County Vaccine Equity Campaigns
Our Community Health Workers (CHWs) provide culturally relevant and linguistically appropriate vaccine outreach, education, and resource connections across Sonoma County. From canvassing markets and churches to hosting health fairs and going door-to-door, our grassroots approach reaches the most vulnerable residents where they are.
